That#s why we search for employees who know that being a true team player means never being As the Director of Case Management, you will facilitate the planning, organizing, implementation and management of all system wide utilization and patient care management functions necessary to support: utilization management discharge planning/coordination successful post discharge convalescence.# Your team will be made up of RN Case Managers, Social Workers, and Interpreters/Translators.#Your role is also critical to the identification of opportunities requiring a continuous quality improvement process in order to refine care delivery processes, develop safety nets for the quality of care provided to pediatric patients or improve inappropriate resource consumption and/or waste. You will work in collaboration with others involved in the creation of a continuum of care for all individuals for whom Cook Children#s Health Care System is the provider of pediatric health care services. # Qualifications Masters of Science in Nursing required Five years of experience managing Utilization Management//Care Management operations within a managed care environment required Proven history of successful physician relations. Demonstrated leadership skills. Excellent oral and written communication skills a must Computer proficiency Significant knowledge of children#s health care needs across the continuum # Licensure, Registration, and/or Certification Basis Life Support (BLS) Certification for Health Care Providers required within 30 days of employment and must be maintained Promise.
