and families. The Healing Shepherd Clinic partners with UGM-TC to
provide education, preventative care, and medical treatment to the
homeless and vulnerable. Come serve with us! About the Role: We are
seeking a highly skilled Nurse Practitioner or Physician's
Assistant to join our team at Healing Shepherd Clinic. As the
Clinic Leader, you will be responsible for providing advanced
nursing care to patients in a variety of settings. You will also be
responsible for educating patients and their families on
health-related issues, as well as providing training and education
to your team. Your work will be critical in ensuring that our
residents and neighbors receive the highest quality care possible.
Minimum Qualifications: Current and valid Nurse Practitioner
license Master's degree in Nursing Experience in Primary Care
Strong communication and interpersonal skills Ability to work
independently and as part of a team Demonstration of leadership or
team management experience. Responsibilities: Assess health status
of patients by: obtaining health and medical history, perform
physical examinations based on age and history, perform or order
preventive and diagnostic procedures, and identify health and
medical risk factors; Develop treatment plans based on assessment
and diagnosis: Develop patient education plans; Perform therapeutic
measures as appropriate, such as cleaning and dressing of
superficial wounds, etc.; Provide appropriate consultation and
referral; Provide follow-up and evaluation of patient status;
Maintain documentation of care in patient records; Ensure
confidentiality of patient records; Provide emergency treatment if
necessary, in accordance with standards of the American Heart
Association of Basic Life Support and Adanced Cardiovascular Life
Support. Collaborating with other healthcare professionals to
provide comprehensive care; Schedule medical knowledge and
immunization events for neighbors and residents; Providing routine
reporting regarding clinic performance to Executive Team and Board
of Directors; Participate in fundraising events; Coordinate
schedules for volunteer medical and dental providers to visit
clinic. Manage Clinic team, providing one on one coaching,
performance management, and scheduling. Skills: As a Nurse
Practitioner, you will utilize your expertise to provide advanced
nursing care to our patients. You will also use your strong
communication and interpersonal skills to educate and counsel
patients and their families on health-related issues. In addition,
you will collaborate with other healthcare professionals to provide
comprehensive care. Familiarity with Health Care Information
Technology is a plus. Your work will be critical in ensuring that
our patients receive the highest quality care possible. Hours:
Typically, the Healing Shepherd Clinic is open Monday - Friday
8:00am - 5:00pm. Some weekend or evening hours may be necessary to
meet emergency needs or fundraising or other types of
