Provide comprehensive outpatient care to adult and senior patients
Diagnose and manage complex medical conditions Maintain accurate
documentation and coordinate necessary referrals Collaborate with
an interdisciplinary care team Participate in quality improvement
and clinical initiatives Required Qualifications MD or DO; active,
unrestricted medical license Board Certified in Family Medicine,
Internal Medicine, or Geriatric Medicine Minimum 2 years of
experience in value?based care or high?acuity geriatric populations
Commitment to patient?centered, outcomes?driven care Participation
in TB screening Preferred Qualifications Training in Family
Medicine, Internal Medicine, Med?Peds, or Geriatrics Experience in
senior care or value?based care settings Proficiency with EHR
systems Strong communication and team collaboration skills
Experience supervising NPs/PAs Additional Details Full?time,
outpatient position (Monday–Friday, 8 AM–5 PM) Competitive
