education to over 2,000 people living with HIV and prevention

Reporting to the Client Services Manager, staff provides ongoing
patient navigation services to persons diagnosed with HIV/AIDS and
their significant family members. This positions clients are
participants in the AfterShock youth program. Duties include: the
completion of helping clients move through the complexities of the
healthcare system, providing referrals and follow-up for needed
services, coordinating services, scheduling enhanced services such
as Life Skills, Employment Skills, and Budgeting, and advocating on
behalf of each client. PRIMARY RESPONSIBILITIES: -Provide
benefits/entitlement counseling and referral to health care
services to assist eligible clients to obtain access to other
public and private programs for which they may be eligible.
-Initiate referrals to agency and community service providers, as
necessary, and coordinate activities in order to provide an
integrated system of service delivery. -Coordinate enhanced
services for AfterShock youth participants. Develop a monthly
calendar of programs and services related to life skills,
employment skills, and money management. Work closely with the
AfterShock care team to improve overall health outcomes for the
youth participants. -Develop an effective working relationship with
area clinics and doctors to be able to attend medical appointments
with clients as appropriate.-Follow-up on care plans generated by
case managers to provide advice and assistance in the areas of:
health/mental health, social support, substance abuse,
financial/benefits, legal, cultural/language, employment, and minor
dependents. -Work cooperatively with medical providers, medical
case management, and outreach providers to recapture individuals
lost to care. -Provide informal counseling and support in areas
such as: Activities of Daily Living, Use of Resources, and Coping
Strategies. -Document service delivery and maintain client records
and confidentiality in accordance with AOC Policies & Procedures.
-Attend and participate in agency related meetings, retreats, work
sessions, and events as requested. -Perform other related duties as
assigned by Client Services Manager and Client Services Director.
-Complete required Department of State Health Services (DSHS)
trainings within 3 months of hire date. Complete 15 hours of job
related trainings per year. QUALIFICATIONS: Bachelor's degree in
Human Services or related field and 1 year experience; or 2 years
experience working in area of social services, including volunteer
coordination; working knowledge of HIV/AIDS; computer literate;
excellent written and verbal communication skills knowledge of
grief and bereavement; knowledge of medical treatments available
for HIV/AIDS; knowledge of family systems; knowledge of
cultural/ethnic issues; knowledge of addiction process and
behavior; knowledge of community resources; excellent written and
verbal skills; participate in ongoing continuing education
activities; knowledge of gay, lesbian and bisexual issues; ability
to work with diverse populations; ability to complete assigned
tasks in a timely manner; ability to effectively handle
crisis/emergency situation; demonstrates a non-judgmental attitude
toward clients, staff and volunteers; ability to discuss
sex/sexuality issues in an appropriate manner with clients ability
to adapt quickly to change; valid Texas drivers license and proven
